Human Hair
Did you know the number of hairs an individual has is already determined at birth? In boys, beard and other body hair becomes thicker during puberty because of male hormones. New hair is not created, rather hormones simply change the hair’s texture. Individual hairs fall out after growing 3-5 years, and in that spot, new hair grows after 3 months. Hair, therefore, continually repeats the cycle of growth - regression -relaxation. Caucasions have 100,000 - 150,000 hairs, on average. About 100 hairs fall out each day, and the same numbers grows, maintaining the approximate number of hairs.
About Hair Loss
Male Pattern Baldness (Androgenic Alopecia)
Balding is an inherited, autosomal dominant trait. While the genes that cause baldness have yet to be discovered, it is known that several genes are involved. The probability of passing those genes to an offspring is 50%. In addition to genetic makeup, other factors contribute to hair loss. These include male hormones, stress, diet and aging. Although balding is caused by male hormones, it is not triggered by the general male hormones of testosterone, but rather by DHT, a substance where testosterone is metabolised by a specific enzyme. Finasteride, currently used for baling, inhibits this enzyme.
When one is bald, does hair stop growing once it falls out?
For patients with androgenic alopecia, hair does regrow once it falls out; however, new hair grows thinner and its growth stage shortens. While normal hair falls out after about 3 years, the hair of a bald individual falls out after a year or less. The new hair is thin and ultimately becomes softer hair. As balding progresses, the hair becomes thinner while the sebaceous gland becomes larger with more secretion. These enlarged glands are only a result of balding, not its cause.
Different Treatments for Male Pattern Baldness
Drugs for Baldness
As drug therapy is especially effective at initial stages of hair loss, early diagnosis is all the more important. Hair loss can be minimised with early diagnosis and adequate drug treatment. Because drugs for treating hair loss must be used for a long period of time, their safety is extremely important, There are two main drugs that have been approved by the US FDA. They are monoxidil and finasteride.
Minoxidil
Minoxidil is a hair growth stimulator originally developed as a treatment for hypertension, but it had side effects of growing hair. While Minoxidil’s hair growth mechanism is fairly unknown, it is most effective on the male crown. The drug is applied daily and takes about 6 months for its effects to be seen. The recommended formulation for men is 5% and for women is 2-3%, to avoid thickening of the fine, soft hair on face.
Finasteride
Finasteride inhibits the production of DHT, the cause of baldness and enlarged prostate, by retaining the function of the necessary enzyme. Finasteride was originally used to treat enlarged prostate, and its effect on hair growth was observed. Finasteride does not thicken existing hair rather it thickens new hair growth. Like minoxidil, Finasteride takes at least 6 and up to 12 months to produce the effects. Some also worry taking Finasteride lowers sexual performance. Whilst the reduction of sexual desire it a topic of much concern, these symptoms are rare, seen in only 2-3 out of 100 men taking Finasteride. Physiologically, men over age 40 experience reduced sexual performance, and they believe it is due to Finasteride.
Quasi-drugs
Although Minoxidil and Finasteride have been proven effective for hair loss treatment, their share of the market is insignificant. The market for quasi-drugs or food products are much larger. Most of these products are applied hair growth stimulators or shampoos, and their main ingredients are often plant extractions that may inhibit the enzyme involved in balding.
Diet
Westerners have 5 times the incidence of baldness than other ethnic groups such as Asians. Heredity plays a central role but diet also plays a factor. We have seen baldness increase as the human diet becomes more westernised. E.g. Vegetables and fruits which contain vegetable estrogen, flavonoids, and unsaturated fatty acids may inhibit the creation of DHT.
Wig
Wigs are a popular method of replacing hair and probably a simple alternative for those with serious baldness. While wigs instantly change one’s appearance, the hairlines may be unnatural, and wearers often complain that wigs are uncomfortable, especially when travelling or playing sports.
Hair Transplant Surgery and other Surgicial treatments
Hair transplantation is a popular procedure for male hair loss. Some patients question whether transplanted hair may fall out. In male-pattern baldness, the problem is not with the top of the head (the field), rather with the hair that grows there (the tree). With Hair Transplants, hair from the back of the head are transplanted into the field where it may grow with its grow qualities intact. In the earlier days, the early punch method of hair grafting was unpopular with patients because of its unnatural appearance. The scalp became bumpy and hair sprouted like bristles on a toothbrush. Later, follicular unit transplant, minigrafting and micro hair grafting, were developed. This leaves fewer traces of surgery and creates a more natural look. Follicular unit transplantation is very popular as it tends to create natural results. Other surgeries have been tried for androgenic alopecia. They have included bi-lobe temporal flap surgery, free flap surgery, and scalp reduction. Most procedures have unnatural-looking results or unacceptable adverse effects and thus are considered out of date.
